Diabetes mellitus commonly known as diabetes is a metabolic disorder that causes high blood sugar. It is a clinical syndrome which is characterized by hyperglycemia due to absolute or relative deficiency of insulin.

Insulin is a hormone which is produced from the beta cells of pancreas and responsible for the metabolism of sugar and fat in our body. With diabetes, the body either doesn’t make sufficient insulin or can’t effectively use the insulin the body makes.

PREVALENCE- in 2020, according to the International Diabetes Federation (IDF), 463 million people have diabetes in the world and 88 million people in the Southeast Asia region. Out of this 88 million people, 77 million belong to India. According to IDF, the prevalence of diabetes in the population is 8.9%.

TYPES OF DIABETES: – Three major type of diabetes can develop. These are

  1. TYPE 1 DIABETES OR INSULIN DEPENDENT DIABETES– this is also known as juvenile diabetes; this type occurs when the body fails to produce insulin requiring replacement therapy. It is a T- cell mediated autoimmune disease leading to progressive destruction of the insulin secreting beta cells.
  2. TYPE 2 DIABETES OR INSULIN INDEPENDENT DIABETES: – in this type there is a combination of resistance to the actions of insulin followed by impaired pancreatic beta cell function, leading to ‘relative ‘insulin deficiency. This means, the body makes insulin but cells in the body do not respond to it as effectively as they once did.
  3. GESTATIONAL DIABETES: – it is a hyperglycemia that is first recognized during pregnancy. It is more of a risk for the baby than the mother. It is important to control this, to protect the baby’s growth and development. It usually goes away after birth.

SYMPTOMS OF DIABETES: – the general symptoms of diabetes are: –

  • Polyphagia (increased hunger)
  • Polydipsia (increased thirst)
  • Increased frequency of urination (polyuria/nocturia)
  • Blurred vision
  • Extreme fatigue
  • Delay in wound heeling
  • Urinary tract infections
  • Dry, itchy skin
  • Irritability, poor concentration

RISK FACTOR OF DIABETES:-

TYPE 1 DIABETES – this usually start at childhood. The probable factor responsible for its development are

  1. Family history
  2. Any pancreatic disease
  3. Any infection like mumps, congenital rubella, cytomegalovirus etc.

TYPE 2 DIABETES: – in this type, the body can’t use the insulin it makes. This is called Insulin Resistance. The main factors leading to the development of type2 diabetes are:

  1. Being obese or overweight
  2. Sedentary life style
  3. Family history of diabetes
  4. Polycystic ovarian syndrome- women with PCOS have a higher risk.
  5. Having a high-density lipoprotein (HDL) cholesterol level.
  6. A history of high blood pressure.
  7. Having history of gestational diabetes
  8. Being more than 45 years of age.

CRITERIA FOR DIAGNOSIS OF DIABETES MELLITUS: – According to European Association for the study of diabetes and the International Diabetes Federation, following are the criteria for the diagnosis of diabetes: –

  • Symptoms of diabetes plus random blood glucose concentration >_ 200 mg/dl, or
  • Fasting plasma glucose>_ 126mg/dl, or
  • HbA1c >_6.5% or
  • 2 hour plasma glucose>_200 mg/dl, during an oral glucose tolerance test.

COMPLICATIONS OF DIABETES:-

  1. Diabetic Retinopathy- cataract, impaired vision.
  2. Diabetic Nephropathy- renal failure.
  3. Peripheral Neuropathy- sensory loss, motor weakness.
  4. Autonomic Neuropathy- postural hypotension, GI problems like gastroparesis, altered bowel habit.
  5. Diabetic Foot
  6. Heart diseases, heart attack, stroke
  7. Depression

MANAGEMENT OF DIABETES:-

Of all the cases of diabetes, approximately 50% can be controlled adequately by diet alone. As it is a major life style disorder affecting the people globally, slight alteration in lifestyle may avoid the future complications. Some of the changes that can be fruitful are as follows:-

  1. Healthy eating habits: – Eat small meals every 2-3 hours regularly instead of consuming large meals. Avoidance of refined carbohydrate and restriction of carbohydrate to 45-60% of total energy intake is recommended. The intake of total fat should be restricted to <35% of energy intake. Eat fiber rich nutritious foods like whole grains, pulses and reduce the intake of oil

People with diabetes are advised to limit sodium intake.

  • Weight management: – being obese or overweight will invite lot more problem to body. So maintain a healthy weight by maintaining your eating pattern. Always keep a look on quality and quantity of your intake. 
  • Exercise: – All patients with diabetes are advised to do physical activity like walking, gardening, swimming or cycling etc for at least 30 minutes per day. Prefer to use stairs rather than using elevators or escalators.  Avoid sitting in same position for longer duration at workplaces, take short break.
  • Practice Yoga- practicing this for 20-30 minutes a day will keep your mind calm and body relax. This can be done at home if you cannot go out for physical activity.
  • Alcohol reduction: – avoid intake of excessive alcohol.
  • Smoking: – cessation of smoking is needed.

What I feel according to my clinical experience and being doctor diabetes in today’s time is increasing due to BITTERNESS IN RELATIONS or LACKING SWEETNESS IN RELATIONS. SWEETNESS in the form of understanding, love, respect, intelligence etc. which patient lost in life but want get diabetes. Most of the population and doctors will not agree with me as all of us read and feel it as a metabolism disorder but what I have felt as time is passing bitterness in life’s of people are adding and so is diabetes .

 Because of the high pace life, stress is the free bonus which is lurking in everyone’s life. The body can not deal with the stress and as a result manifest itself with variety of ailments and diabetes is one of them.

The main conflict associated with this is MALE RESISTANCE CONFLICT or a FEMALE FEAR DISGUST CONFLICT.

A Resistance conflict is a strong opposition against a person like it can be for parent, step parent, sibling, relatives, teacher, colleague etc. It can be against a situation like at work place you are supposed to do the things that you don’t want to do but just in fear of losing that job you are forced to do that, you want to come out from any relationship but just because of some factor you are sustaining that etc. These resistances can be against any institution, like school, office hospital etc. The person is forced to do the things that was against one’s will and all these things will lead to stress and the development of disease.

Children suffer the conflict at an early age, when they resist day care, kindergarten, or school, or when they strongly oppose what they are told to do.

In case of female fear disgust conflict is present. This conflict can be due to some bad memory that make her frightful by the thought only. Like if there is history of sexual harassment, then this can lead to development of this conflict.
